Henry, regarding a reference for the Maryland experience, you may want to look at: Murray, Robert, “Setting Hospital Rates to Control Costs and Boost Quality: The Maryland Experience,” Health Affairs, Vol. 29, No. 5 (September/October 2009). A non-academic synopsis is offered by the WSJ, Business Section, Monday September 14, 2009. The Center for Studying Health System Change has several newspaper articles that provide more recent information; you may also want to look at Paul Ginsburg's Research Brief #16 at HSC.

I'd like to see comments on how the State of Maryland is dealing with cost and access. It's the only state left with all-payer rate setting for hospitals. Who is benefiting?

Very much agree with Hrant's comments above, especially regarding need for transparency - although it must be about more than just benefit comparison. When the price of colonoscopy can vary by 100's of % among providers (Uwe Reinhardt's report of calling 50 sites- getting 50 prices), if the client (employee?) gets it free, except if there's a polyp cause then it's treatment) we're talking a lot more than widget comparison.
How is this handled in Europe? Standardized reimbursement? Or...?
